In the “Death of Captain America” story, the Red Skull’s plan was that Crossbones would shoot one of the US Marshals escorting Captain America into the courthouse, and in the ensuing chaos, mind-controlled Sharon Carter would shoot him with a time gun (comics), which would cause Cap to become unstuck in time. It was all part of the Red Skull’s plan to (once again) take over Captain America’s body.
Minor correction, Maria Hill was a Bendis creation, not Millar. She first showed up in an early New Avengers, but I think she was originally created for Secret War first. Since Secret War took forever to come out, her true debut took a while.
